Travel Reimbursement Form

Cost of transportation (up to $255 per bus) is eligible to be subsidized by Hamilton County Recycling and Solid Waste District for those schools registered in the District’s Recycling Assistance Program (RAP).

To apply for the trip:

Step 1: Print, complete and return this signature form.

Step 2: Complete the below pre-registration form. Hint: Print form for your records, prior to submitting.

Step 3: Student Reflections must be submitted within 10 days of field trip.

  • Prepare students before the trip with expected learning outcomes.
  • Have students debrief in whole group or with a partner focusing on what they learned; why it is important, and how they can use this information in their lives.
  • Students should be given time to reflect and the reflections may be submitted in any number of mediums. For instance, the students may opt to create posters, write poems, create a song, write a letter to their local representative, create a video, etc.

Field trips are limited to not more than four trips per school, per year.

The District will reimburse up to $255 per bus to successful applicants. Contact your transportation manager or provider for a quote.

I understand I am responsible for scheduling any required bus transportation to and from the field trip. 

If the trip is cancelled for any reason, I must notify the transportation provider at least two hours prior to the school pick-up time or I may be responsible for late cancellation expenses charged by the bus provider which are not subject to reimbursement. My principal is aware that I am applying for this field trip and has agreed to allow my students to miss class time to attend. I understand that a minimum of five weeks is required for processing reimbursements and reimbursement requests must be submitted within 90 days of the trip, or my right to reimbursement will be forfeited.
The undersigned on behalf of themselves and their participating school, understand and agree that neither the Hamilton County Recycling and Solid Waste District nor its principles or employees shall be held liable for damanges or injuries that may in way arise out of the field trip or the transportation.
